Referral Instructions

Bloom Recovery Center accepts referrals from:
  • Providers and healthcare professionals
  • Probation officers and case managers
  • Family members
  • Individuals seeking help for themselves
To make a referral, please include:
  • Client’s full name and contact information
  • Referral source (your role/relationship)
  • Any relevant treatment history or documentation
Our team will follow up promptly to ensure the individual receives the care and support they need.
